Kidney Transplant is the best form of renal replacement therapy for people with end-stage renal disease (ESRD), in comparison to dialysis. Its advantages over dialysis include higher patient survivals, decreased hospitalization rates and better life quality. However, allograft failure over time after transplantation is still a significant problem. What are the causes?
The major two causes of renal transplant failure are: patient death after transplantation and allograft failure due to immunological or non-immunological causes or a combination.
1. Death after transplantation
Though patient's survival following a transplant is higher than that for ESRD patients on dialysis, kidney transplant recipients are still at risk for death from cardiovascular disease because of their prior history of kidney disease, high blood pressure, excess atherosclerotic vascular disease and other comorbidities like Diabetes and infections.
2. Allograft failure due to immunological or non-immunological causes or a combination.
▪ Immunological causes: acute rejection and chronic rejection.
▪ Non-immunological causes: surgical problems, renovascular thrombosis, nephron injury due to various causes including ischaemia-reperfusion and nephrotoxicity from calcineurin inhibitors (CNI), hypertension, and hyperfiltration.
▪ A significant proportion of allograft failure can be caused by recurrence of original disease or Chronic Allograft Nephropathy (CAN).
Generally, conventional treatment options for kidney transplant failure are hemodialysis, peritoneal dialysis or re-transplant. Under these therapies, patients still live with lots of sufferings.
