Routine Exams and Tests for End Stage Renal Disease (II)

2012-10-06 16:10

III. Renal Function Test

1. Glomerular Filtration Rate (GFR)、creatinine clearance rate (Ccr)

Currently, patients can receive ECT to test the GFR. In End Stage Renal Disease, both of GFR and Ccr will become decreased. If the GFR<25ml/min or Ccr<10ml/min, patients have enter ESRD stage.

2. CHZO

People with ESRD may have abnormal CHZO.

3. Phenol red excretion test and urine concentration dilution test

For adults, 15 minutes after they inject the phenol red, the excretion amount should be more than 25%. 2 hours later, the excretion amount should be more than 55%. Children often have higher excretion, while lower for elderly persons. In ESRD, the amount will decrease.

Urine concentration dilution test, no matter Mosenthal method or Vol Hard method, the difference of the highest and lowest proportion should be no less than 0.009. In ESRD, the test may be below 0.009.

4. Other tests

These tests mainly include radiography, kidney biopsy, nucleo-renogram, scanning of kidney, etc.

Nucleo-renogram: the diagnosis of ESRD causes is non-specific.

Scanning of kidney: in case of renal atrophy, the scanning may show shrunken kidneys, sparse radioactive distribution, and asymmetrical distribution.

Kidney biopsy is convenient, but the result may lead to mis-diagnose of the disease.

The above is a general introduction of routine exams and tests for ESRD. With an early diagnosis of the disease, prompt treatment can be adopt to guarantee better outcome.
