High blood pressure impairs the kidneys mainly by damaging renal small vessels.Long-term or severe hypertension can cause pathological changes in renal small vessels, which involve nephrons. Renal Arterioles Sclerosis
Blood pressure rise of the whole body can cause renal bloodstream regulatory function disorders and high blood pressure of glomerulus, resulting in glomerular high perfusion status.Although afferent glomerular arteriole contracts due to primary hypertension, the contraction intensity is far from enough compared with elevated blood pressure; as a result, systemic hypertension passes into glomerulus, leading to functional and organic changes and damages of renal arterioles and glomerular capillary loops. As the blood vessel endothelium is impaired, many elements in plasma can infiltrate and deposit in vascular walls. Besides, mechanical injury can conduces to the collagen synthesis, causing glassy degeneration of renal arterioles and capillary loops, leading to focal segmental glomerulosclerosis.
Pathological Changes of Renal Tubules
Pathological changes of renal tubules are caused by renal ischemia. Because renal tubules are more sensitive to renal ischemia than glomerulus, and with high perfusion of glomerulus due to high blood pressure, the burden of renal tubules increases in order to maintain normal GFR(glomerular filtration race). These all are more likely to deteriorate the impairment of renal tubules.
The damage to glomerular artery and nephrons due to hypertension is focal, but kidney function decline in the late term is progressive. On one hand, high blood pressure can impair kidneys, and kidney damage aggravates hypertension; they two form a vicious circle, and cause pathological changes in normal small artery and nephrons. On the other hand, hypertension induced nephrons injury can cause compensatory high perfusion and high filtration in remaining normal nephrons. If high blood pressure continues, the remaining nephrons will reduce progressively and lead to renal function decline in progression.
