Medullary Sponge Kidney Diagnosis: Tests and Exams

Kidneys are consisted of small tubes called tubules. These tubules are responsible for the flow of urine throughout the kidney. In Medullary Sponge Kidney, cysts with sac like structures develop in an area known as the medulla, which causes the appearance of a porous or sponge like structure. Then, what tests and exams are involved in the diagnosis of Medullary Sponge Kidney?


Microscopic hematuria may be seen. Pyuria is seen with urinary tract infection. Or the urinalysis may be normal; dipstick positive for leukocytes, nitrates, blood; microscopic analysis positive for WBCs, RBCs, or bacteria; alkaline pH.

Urea and creatinine

People with Medullary Sponge Kidney rarely have high urea and serum creatinine caused by obstructive stones or recurrent pyelonephritis.

Serum electrolytes

It involves test of calcium, chloride, potassium, phosphate, magnesium, phosphorus, and uric acid. These levels may be normal or elevated.

Renal ultrasound

It may be ordered for non-specific abdominal pain, or if urinary tract obstruction is suspected. Result: papillary densities observed irrespective of presence of medullary calcification.

Intravenous urography (IVU)

An x ray called intravenous pyelogram( IVP) may be ordered by the doctor to help confirm the diagnosis. In this test, a dye is injected into vein. It can show any blockage in the urinary tract. If cysts are present, they will show up as clusters of light in the IVP.
