Purpura nephritis is one of serious complications of Henoch-Schönlein Purpura. It is featured with protein or blood urine in urine. In some cases, the patients may have no abnormalities in urine. What is the prognosis of purpura nephritis and how to improve its prognosis?
Generally speaking, children usually have better prognosis than adults. Heavy proteinuria and high blood pressure are two major risk factors in worsening renal function and accelerating renal function decline.
According to some statistics, about 40 percent patients with stage Ⅳ or worse stage will develop renal insufficiency. However, the patients with stage Ⅲ or better stage, the incidence of renal insufficiency reduce significantly, only 3%.
In addition, Purpura Nephritis occurs due to the deposition of immune complexes in glomeruli. The position of deposition also can affect its prognosis. If the immune complexes only deposits in glomerular mesangial area, it usually indicates a good prognosis. However, if the immune complex deposition is found in subepithelial and subendothelial area and mesangial area, this condition is associated with a worse prognosis of purpura nephritis.
If purpura nephritis is not controlled well enough, it is easy to deteriorate into nephrotic syndrome in a short time and eventually renal failure. So how to improve the prognosis of purpura nephritis and prevent it from developing into renal failure?
1. Control blood pressure within 130/80 mmHg. A low-salt diet along with medications like ARB drugs and ACE inhibitors can help a lot.
2. Treat proteinuria effectively and reduce the proteinuria to less than 1g/24 h. It can delay the progression of the condition significantly. ARB drugs and ACE inhibitors can help control proteinuria. Additionally, hormone and immunosuppressive dugs are prescribed to treat proteinuria.
3. Another important treatment is to eliminate the immune complexes in glomeruli. Micro-Chinese Medicine Osmotherapy can degrade the immune complexes and dissolve them in blood. Eventually, they will be filtered out of body through systemic circulation.
4. However, if not the immune disorder is not corrected, more immune complexes will be produced. Therefore, immunotherapy should be applied to correct immune system so as to stop abnormal immune reaction.
After the above systemic treatment, purpura nephritis can be controlled effectively. Thereby, the patients can get a good prognosis.
